Improving information sharing for safer housing moves

In 2024, the Norfolk Safeguarding Adults Board published a SAR for Adult P.  A white male, Adult P was 37 years old when he died in August 2021 at his home in Norwich.  He lived alone with little information known about his immediate family.  Adult P experienced a great deal of trauma as a child; he was abused sexually and physically.  Suffering a back injury in 2019 resulted in limited mobility and Adult P used a mobility scooter.  He had a history of falls, mental ill health, drug and alcohol use, and concerns were raised that he was allowing people to come into his home who were threatening him.

The review identified that when Adult P moved between housing providers, background and safeguarding information was not shared effectively.  To address this, housing providers and district councils have been reviewing working arrangements to ensure a more effective handover of background and safety information, particularly for high-risk clients when they move between providers.  To support this, an information sharing agreement has been put in place.

Colleagues at South Norfolk and Broadland District Council have shared the agreement with their participating organisations to sign (there were 38 in total, including district councils, registered providers and supported housing providers).  They have already had a good number of returns and are following up with the remainder.  You can read more about their success on South Norfolk and Broadland District Council's website.
